School Nurse

Welcome to the school nurse's page. This page is where you will find access to medical forms, action plans, and upcoming information. If your child has a serious medical condition, such as seizures, diabetes, asthma, heart issues, or food allergies, please complete an action plan and turn it in to me or their school at registration with their other paperwork. Also, medication authorizations must be filled out by your physician if your child will be taking any prescription or over the counter medication while at school.  Please refer to the immunization guide for grades requiring immunizations. See below for health requirements for specific grades. (Please note that sports physicals do not count as health exams).  If you are new to the district, please contact me for further specific needs.  I hope you have a happy and healthy school year!!!

Requirements:
Pre-K:  Physical exam, immunization record with current vaccines, lead screen
Kindergarten:  Physical exam, immunization record with current vaccines, lead screen, eye exam by optometrist or ophthalmologist, and dental exam
Second Grade:  Dental Exam
Sixth Grade:  Physical Exam (Sports exams do NOT meet requirements), immunization record with current booster vaccines, dental exam
Ninth Grade:  Physical Exam (Sports exams do NOT meet requirements), immunization record with current vaccines, dental exam
Twelfth Grade:  Meningitis Vaccine 


The state deadline for submission of required health records to the school is October 15 of each year.  If your student does not have their health information turned in by that date, they are excluded from attending school until it is received.  (Dental exams are not due until May 15.)

If your child has never attended an Illinois school and is new to the Unit 40 district, they must submit an Illinois physical exam and eye exam regardless of grade.  You have 30 days to meet this requirement after registering your child.
